2024 Jon Hackenyos Practice Excellence Award Winner

Infinity Internal Medicine in Chandler is Arizona Care Network’s Jon Hackenyos Practice Excellence Award winner for the 2024 performance year.

This annual award honors a local primary care practice which achieves the highest performance within ACN’s quarterly incentive program, as determined upon completion of the prior year’s fully measured program.

In December 2022, ACN’s physician-led Board of Managers established the award recognizing high-performing primary care practices within the network, named in honor of Dr. Hackenyos: The former ACN Board Chair, Network Committee Chair, and
co-founder of Ocotillo Internal Medicine Associates who passed away in December 2021.

Led by current ACN Board Member, Dr. Megan Jhaver, Infinity Internal Medicine will receive the award during a celebration at its Chandler practice in July.

On behalf of ACN and our Board of Managers, congratulations to Infinity Internal Medicine, and all participating practices, on your hard work, dedication to patient care, and success!

Coming in 2025: Member Care Scorecard Website, App

Arizona Care Network is excited to introduce a new online and digital resource designed to support your patients’ journey toward better health: the Member Care Scorecard.

This easy-to-use digital tool can help members stay updated with his/her preventative health and maintenance goals by tracking important quality care measures from their fingertips, including:

    • Annual Wellness Visits
    • Cancer Screenings
    • Blood Pressure control
    • A1C control, and more.

A member’s personalized view of their care Scorecard encourages proactive engagement with their health. It also supports stronger relationships with providers through quality care that is easier to understand, track, and complete.

When a member logs into the Scorecard, a dashboard shows which care measures are completed and which are still outstanding. A member can request appointments, submit visit confirmations, and access helpful explanations about the importance of each measure.

Upon completion and approval of privacy, security, and compliance testing later this fall, the Member Care Scorecard is scheduled to be available Q4 of 2025 via website, and as an app through the Apple and Google Play stores.

Whichever device a member uses, the Care Scorecard will be a secure and user-friendly experience at their fingertips (screenshots below).

Once launched, ACN plans to further enhance the platform with reward incentives for completing certain care benchmarks, along with educational tools and reminders to help members achieve their best health.

Watch for further communications about the Care Scorecard website and app for both providers and members in the coming months.

MSSP Enhanced Follow-Up Notification Delivered to Beneficiaries

As part of CMS’s participation requirements within the Medicare Shared Savings Program (MSSP) Enhanced ACO model, notification follow-up letters were sent by ACN to beneficiaries attributed to an ACO-participating practice.

The letter and notification serve as a reminder that each member received a similar letter in early 2025 in both English and Spanish. The letter and notification explain the MSSP Enhanced program, its benefits, and no-cost services available to all members (including Care Coordination). It also includes privacy and contact information.

Per CMS guidelines, a verbal or written follow-up communication (written, email, verbal, telephonic) is required no later than the next visit following the initial beneficiary notification each year, or, if no subsequent visit, within 180 days of the initial beneficiary notification.

ACN’s Urgent Care Provider Website Upgrades Now Complete

To improve ACN’s Find-a-Facility modal within our website and encourage member cost savings and convenience in avoiding preventable Emergency Room usage, several upgrades to the search tool are now complete.

Among the website’s search tool improvements:

    • “Urgent Care” is indicated as part of the “Find-a-Facility” identification label (see image below):

    • NextCare Urgent Care locations within search results include the NextCare logo plus a purple Schedule button that
      re-directs a user to that NextCare location’s online scheduling platform (see below images):

    • If a user selects a specific NextCare location (blue text within the Find-a-Facility search tool), the location will appear on a map, and the Additional Locations listings will automatically show the next 10 closest locations to the selected location, in order of miles (see below image):

Additionally, a separate Pharmacy resources section was also added.

Quality: Screening Season for Breast Cancer, Cervical Cancer

With summer in full swing, preventative screenings for adult women of all ages should take center stage.

These screenings among your female patient population not only close important Quality care gaps, but can save time, medical costs, and even lives.

The No. 2 most common cancer among women after skin cancers, breast cancer mammography screenings should begin at age 50 (or earlier depending on a patient’s lifestyle factors and family history).

Cervical Cancer Screening

Quality: Summer is Annual Wellness Visit, Well-Child Visit Season

Summer is here to stay for the next few months, which means it is the perfect time to schedule Annual Wellness Visits (AWV) and other important checkups.

This (typically) no-cost annual visit with a primary care provider is an essential part of keeping your patient population healthy.

These visits are a perfect opportunity to proactively improve health outcomes through conversation, vaccinations, and early detection screenings for breast cancer and cervical cancer.

  • Dignity Health St. Joseph’s Hospital and Medical Center in Phoenix now offers Liver Histotripsy, a non-invasive, image-guided therapy for the treatment of liver tumors. Histotripsy technology uses focused ultrasound waves to mechanically break down precisely targeted liver tissue, including tumors, without the need for incisions or radiation.   • Abrazo Health’s new 3-story medical building in Buckeye officially opened in mid-June. The 60,000 square foot building houses a variety of medical services: Cardiology, orthopedics, spinal care, sports medicine, internal medicine, urology, physical therapy, and more. You can watch the building’s construction and grand opening.
  • Abrazo Health’s annual Heart Walk is scheduled for Nov. 15 at Goodyear Civic Square. The non-competitive 3-mile walk raises money each year to fight heart disease and stroke. You may register as an individual, with your family, or as a group of people. More information and registration are available here.
  • Mercy Care offers eligible members a no-cost Smartphone for talk, text, data, and apps, with nationwide coverage. More information is available in English and Spanish.
  • Dignity Health Chandler Regional Hospital, Mercy Gilbert Medical Center, and Arizona General Hospital – Mesa, awarded more than $540,000 in community grants to 36 local nonprofits across 11 projects this spring. The grants program is part of the CommonSpirit Health Community Health Improvement Grants program, which is dedicated to improving the health and well-being of vulnerable and underserved populations.   • Phoenix Children’s is supporting a 1-credit CME opportunity (PRA Category 1) is available July 29 at 7:30 a.m. Say What? Diagnosing & Treating the Seven Types of Jargon Overuse is a 1-hour Zoom session hosted by Dr. Michael Pitt, a pediatric hospitalist at the University of Minnesota. The session explores best practices for identifying and avoiding the use of jargon when communicating with patients. More information and the Zoom link are available here.
  • Phoenix Children’s is hosting its annual EMS and Prehospital Care Conference on Monday, Oct.13, at 9 a.m. at the Renaissance Phoenix Glendale Hotel & Conference Center. The program features programs and discussions of pediatric emergency and first responder care, along with technology showcases and best practice exhibitions. Lunch is included and CME credits are available for EMTs, paramedics, and nurses. More information and registration are available here.
