Practice Tools & Resources

Arizona Care Network provides tools and resources for practices to successfully manage the cost and quality of care for their patients.

These services are free-of-charge to the practice. They enable practices to maximize the benefits of joining an ACO and deliver care that keeps patients healthy at a low cost.

Provider and Practice Data

In-network care coordination for all healthcare services improves patient outcomes and ACN’s value-based agreements. In-network referral opportunities are conveniently available through the Find a Doc or by contacting our Care Coordination team. Additionally, your Clinical Performance Consultant is always available to support your practice and patient needs with in-network referral management resources.

Care Coordination

ACN offers clinical support to providers who treat patients with complex conditions. Our Care Coordination team includes registered nurses, behavioral health coaches, social workers, and navigators. This extra layer of support helps patients follow their treatment plan to reach optimum health.

Provider and Practice Data

When you join ACN, you receive access to our secure online provider portal. Our analytics and clinical performance teams regularly upload data to the portal for your practice and your attributed patients.

Data reports available in the portal include:

  • Care Gap Reports
  • ER and Readmission Reports
  • Historical PIP earnings
  • Patient Satisfaction scorecards
  • Patient Segmentation Reports

Other features within the portal include:

  • Quick Access to Clinical Integration Tools (Athena, HIE, etc.)
  • Events Calendar
  • Member Look-up
  • News & Announcement

Additionally, ACN’s clinical performance team will meet with your practice to review patient data and help close care gaps if they exist.

Health Information Exchange

In addition to the state health information exchange (HIE), ACN grants you access to Dignity Health’s HIE so you can efficiently track patient activity within the Dignity Health system. This level of insight helps providers identify vulnerable patients who may be high ER utilizers.

Clinical Performance Consultants

Your Clinical Performance Consultant (CPC) is the primary contact for any questions you may have.

They can assist you with:

  • Payer/insurance questions
  • Referral management
  • In-network utilization
  • Quality outcomes
  • The Provider Rewards Program and other ACN network initiatives

Your CPC can also help guide your practice on:

  • Quality initiatives
  • Identifying patients for care coordination
  • GPRO auditing
  • Delivering on the Quadruple Aim
