The National Association of ACOs (NAACOS) honored Arizona Care Network with a 2019 Innovation Award for Care.Wallet, a provider application built on blockchain technology that displays actionable clinical insights. As a result, providers can more efficiently administrate and coordinate healthcare processes, improving population health.

Today my family began using Care Coordination to coordinate support and services for our son’s orthopedic surgery. I was very impressed with Teresa Olguin, the ACN Care Coordinator who called me this morning. She is an RN. She is very familiar with the additional challenges and complications we face. And in 16 years, she is the first healthcare provider who during an initial contact inquired as to how my son is dealing emotionally with this current health challenge. She also shared, without any further details that would violate HIPPA, that we aren’t the only family with a child who has special needs currently on her caseload.
