Arizona Care Network is bringing you a new way to proactively manage your health: Care Scorecard.
What is Care Scorecard?
Care Scorecard is an app that allows you to track important health activities, like seeing your doctor annually or getting certain lab tests, while earning rewards along the way.
With Care Scorecard, you can:

- Track your recommended health screenings and earn rewards for completing them
- Request for Arizona Care Network to help you schedule a health checkup
- Search for a healthcare provider that is in-network on your insurance plan
- Learn about health topics that are important to discuss with your doctor
- See how your progress compares with others within Arizona Care Network on our leaderboard
- Chat with Scott, our personal health assistant that can help answer your Care Scorecard related questions
How it Works
When you go to the doctor and complete a recommended care task, we will update your Scorecard automatically once the insurance claim is processed. Or, you can let us know by marking the task done. We verify that you completed the task, then update your Care Scorecard. When you complete 5 health tasks within a year, you will earn a reward!
How to Sign Up
Signing up is easy and free. Simply go to MyCareScore.org and register with your name and member ID found on your healthcare insurance card. If you need help setting up your account, contact members@azcarenetwork.org or call (602) 406-7226.

No, Care Scorecard is free for Arizona Care Network members.
How do I know if I am an Arizona Care Network member?
You may already be an ACN member based on the primary care provider you see and/or your insurance benefits.
