Arizona Care Network’s Care Coordination team features health care experts that encompass the full range of health care services, customized to your patient’s personal needs. Care Coordination is an included benefit for patients whose providers participate with Arizona Care Network.

Our Care Coordination team is here to help. We can augment your treatment plan with a care plan that supports your patient’s needs. Our team addresses the social determinants of health and other barriers to help your patient succeed.

The Care Coordination team utilizes several health care professionals including:

  • Navigators
  • Social Workers
  • Behavioral Health Coaches
  • Registered Nurses
  • Population Health Medical Assistant
