The Post-Acute Services Program Manager is responsible for leading a variety of relationships related to post-acute services offered by ACN, including post-acute network design, post-acute care coordination services, post-acute provider relationship management, waiver program implementation and maintenance, post-acute program metric selection and reporting to internal and external stakeholders, as well as hospital system case management program partnerships.
