Program Manager, Post Acute Services

The Post-Acute Services Program Manager is responsible for leading a variety of relationships related to post-acute services offered by ACN, including post-acute network design, post-acute care coordination services, post-acute provider relationship management, waiver program implementation and maintenance, post-acute program metric selection and reporting to internal and external stakeholders, as well as hospital system case management program partnerships. RN Care Coordinator

The RN Care Coordinator (CC) is an individual with significant experience and demonstrated ability to engage providers and care teams in collaborative care , generation of better clinical outcomes, driving higher patient satisfaction with service delivery, and efforts to improve the cost-efficiency and value of service delivery. Sr. Population Health Analyst

The Sr. Population Health Analyst will work with a variety of types of data, including lab, medical (integrated EMR), pharmacy, and claims data. This position will provide the opportunity to grow and learn, as well as multiple opportunities to present to leadership, physicians, and payors.
