Manager, Care Coordination

The Manager of Care Coordination is responsible for the oversight of Care Coordination and Social Services, including, but not limited to: clinical resource management, discharge planning activities, patient advocacy, clinical social work and best practice in medical necessity determination and concurrent review. This position will oversee the department in an efficient manner to ensure timely and compliant care coordination, discharge planning, utilization review and social work interventions resulting in quality patient care. Program Manager, Post Acute Services

The Post-Acute Services Program Manager is responsible for leading a variety of relationships related to post-acute services offered by ACN, including post-acute network design, post-acute care coordination services, post-acute provider relationship management, waiver program implementation and maintenance, post-acute program metric selection and reporting to internal and external stakeholders, as well as hospital system case management program partnerships. Clinical Performance Consultant

The Clinical Performance Consultant is responsible for educating our clinicians and their supporting teams in the ambulatory space on the value based arrangements (VBAs) our network supports, and also advising these teams on techniques to improve their performance on these VBAs. Patient Navigator

The Navigator supports the primary care team with reducing fragmentation of patient care, improves compliance and access to care, supports efforts to reduce or remove treatment barriers, and assists patients in navigating their path through the continuum of care. .
