The Centers for Medicare and Medicaid Services (CMS) created this structure to enable groups of doctors, hospitals, and other health care providers work together using proven protocols and measures designed to enhance care quality, better coordinate patient care, improve efficiency and lower costs. Coordinated care helps ensure that patients, especially the chronically ill, get the right care at the right time, with the goal of avoiding unnecessary duplication of services and preventing medical errors.
ACN serves Medicare members through the Next Generation ACO Model, sometimes called Next Gen or NGACO. This a Medicare model where healthcare providers who share a common vision work together to improve the care provided to Medicare beneficiaries. There are 51 NextGeneration ACOs in the country and Arizona Care Network is the only one in Arizona.
