Medicare and Medicare Advantage Members

Arizona Care Network is an Accountable Care Organization (ACO).

The Centers for Medicare and Medicaid Services (CMS) created this structure to enable groups of doctors, hospitals, and other health care providers work together using proven protocols and measures designed to enhance care quality, better coordinate patient care, improve efficiency and lower costs. Coordinated care helps ensure that patients, especially the chronically ill, get the right care at the right time, with the goal of avoiding unnecessary duplication of services and preventing medical errors.

ACN serves Medicare members through the Global and Professional Direct Contracting Model, sometimes called the GPDC Model. This a Medicare model where healthcare providers who share a common vision work together to improve the care provided to Medicare beneficiaries.

If you are a Medicare member and your primary care provider participates in the GPDC Model, you are already part of ACN! That means you have access to the programs and support ACN provides to our members, like Care Coordination, 24/7 access to ACN’s Concierge Line and access to our network of 5,500 specialists in 1,800+ care locations throughout metro Phoenix.

CMS requires all ACOs to post information about their organization, GPDC Model participation, and list participating providers. You’ll find that information here.
