Global and Professional Direct Contracting

About the Global and Professional Direct Contracting (GPDC) Model

The Centers for Medicare & Medicaid Services (CMS) founded the Global and Professional Direct Model (GPDC) in 2021 as a Model where healthcare providers who share a common vision work together to further improve the healthcare provided to Medicare beneficiaries. The healthcare providers who participate in Arizona Care Network are voluntarily taking part in this Model with the goal of providing better quality care for our Medicare fee-for-service patients. All healthcare providers participating in our GPDC Model have joined with CMS in committing to your needs and preferences, and to emphasize the coordination of your care. You can learn more about this Model by clicking here. As always, you can contact Medicare directly at 1-800-MEDICARE (1-800-633-4227; TTY users can call 1-877-486-2048).
