Through Arizona Care Network’s value-based agreement with Mercy Care, our providers care for nearly 110,000 Medicaid members, and a significant portion of that population is considered at-risk or highly vulnerable. ACN identified more than 9,000 Mercy Care members that have a significant mental illness or behavioral health diagnosis.
To help address the extensive needs of these patients, ACN established Comprehensive Care Clinics that provide integrated primary care and behavioral health services. Key components of this program include:
• 2 fully-integrated clinic locations with primary care and behavioral health care at one site
• 1 virtual location with access to ACN services from any location
• Dedicated staff to care for complex needs
• Embedded Care Coordination team to address needs in real time:
o RN Care Coordinator
o Social Worker
o Healthcare Navigator
o Population Health MA
o Behavioral Health Coach
o Population Health Pharmacist (remote, as needed)
A behavioral health diagnosis can add a layer of complexity to a patient’s treatment plan. These patients often require additional resources, which may be out of scope for some providers. Our comprehensive Care Coordination is a no-cost alternative that can benefit the provider and the patient.
Referring a Patient
When you refer a patient to a Comprehensive Care Clinic, you effectively transfer that patient to the primary care provider at the clinic. This ensures the patient has access to the resources they need while you remove a high-cost patient from your attribution list.
