The Connected Care Arizona Concierge is a one-stop resource to help you find a doctor who’s right for you and your family, answer questions about benefits and claims, verify eligibility, process ID card requests, and more. Connected Care ACN offers a new way to manage your health and supports keeping you well rather than just treating you when you’re ill. With ACN, in-network providers communicate with you and with each other. They work together to maximize your health and minimize your out-of-pocket costs.
This model of care is useful for any patient but it’s especially helpful to those with chronic or complex health conditions like diabetes, obesity, COPD, depression, cancer or even one-time issues that require surgery and/or hospitalization.
If you or a dependent are managing a chronic or complex illness, ACN is here to help. Our Care Coordination team works closely with your doctors and you to help manage your condition with the goal to reduce the likelihood of hospitalization and help you achieve your best health. We can also make sure you’re not alone when making important or difficult decisions about care. It’s like having a personal health concierge.

Registered Nurses – Licensed clinical professionals offering targeted education, case management, disease management, medication management and care transitions
