What is Care Scorecard? 

Care Scorecard is an app that allows you to track important preventative health tasks, like seeing your doctor annually or getting certain lab tests.

These health tasks, such as blood pressure or A1C screenings, help catch issues early, when they’re easiest to treat. Staying current on these screenings helps you stay healthier and avoid complications down the road.

With Care Scorecard, you can: 

  • Track recommended health tasks and see which ones you’ve already completed
  • Search for a healthcare provider that is in-network on your insurance plan
  • Learn about health topics that are important to discuss with your doctor 
  • See how your progress compares to others on our leaderboard 
  • Chat with Scott, our personal health assistant that can help answer your Care Scorecard related questions 

How it Works 

When you go to the doctor and complete a recommended care task, we will update your Scorecard automatically once the insurance claim is processed. Or, you can let us know by marking the task done. We verify that you completed the task, then update your Care Scorecard.

The first 50 Intel Connected Care participants who complete 5 health tasks will receive a $50 gift card!

How to Sign Up 

Signing up is easy and free for Intel Connected Care members.

Simply go to MyCareScore.org and register with your name and member ID found on your healthcare insurance card. If you need help setting up your account, contact members@azcarenetwork.org or call (602) 406-7226.
