Becker’s Hospital Review, a respected national healthcare business publication, recognized Arizona Care Network as one of the “ACOs to Know” in 2017 and 2018
In 2018, ACN was named Leader of the Year in Public Policy – Healthcare by the Arizona Capitol Times. The award noted ACN’s success in delivering high quality care to challenging Medicaid population through an innovative care model and careful disease management, while reducing the overall cost of care by $14.4 million over two years.
AZ Business Magazine honored Dr. David Hanekom, ACN’s retired chief executive officer, at its 2018 Healthcare Leadership Awards for his excellence in healthcare innovation and success in advancing the Quadruple Aim: improve the health of our population, reduce the cost of care, improve the patient experience and drive provider satisfaction.
The National Association of ACOs (NAACOS) honored Arizona Care Network with a 2019 Innovation Award for Care.Wallet, a provider application built on blockchain technology that displays actionable clinical insights. As a result, providers can more efficiently administrate and coordinate healthcare processes, improving population health.
Care Coordination is available to any ACN member but it’s especially helpful to patients who have complex care needs, see multiple providers or are managing a new health concern. The following are real testimonials and partners:
I was very satisfied with my care. My surgery was very complicated. I required two separate surgeries, therefore I spoke with many doctors, nurses, aids, therapists, care workers, etc. and I moved locations several times. It’s hard to remember who does what. [My ACN Navigator] helped me with the details so I could get all the special care I required at the hospital, skilled nursing facility and rehab. So again I say “thank you” for good care and consideration. I am in my 80s and adjusting to an unexpected, drastic change in my lifestyle.

Bill French (ACN health coach) has been an invaluable aid in assuring my mental health care needs are addressed, not only by my PCP but by my mental healthcare provider.
When I transitioned providers, I had some issues so Bill came to meet me at the new clinic to assure a smooth transfer. With my new doctor’s help I hope to get my pain under better control. In this area Bill has also been of assistance in finding a suitable pain management clinic, discussing treatment options, and interfacing with my PCP.
In an effort to support me through this difficult time of clinical depression, Bill has been like a steady rock that I can depend on. In many ways he is like a “warm line” that knows my issues. He has been of great assistance not only in assuring I receive quality care but also as a trusted counselor helping me in times when I had no others to stalk to. It is a comfort to know that Bill is only a phone call away and will be there to talk about problems that are overwhelming me at the time.

Today my family began using Care Coordination to coordinate support and services for our son’s orthopedic surgery. I was very impressed with Teresa Olguin, the ACN Care Coordinator who called me this morning. She is an RN. She is very familiar with the additional challenges and complications we face. And in 16 years, she is the first healthcare provider who during an initial contact inquired as to how my son is dealing emotionally with this current health challenge. She also shared, without any further details that would violate HIPPA, that we aren’t the only family with a child who has special needs currently on her caseload.

I’ve always been impressed with how effectively our teams work together to ensure that the people we collectively serve are getting the best care and experience.
