About the Next Generation Accountable Care Organization (ACO) Model
The Next Generation ACO is a Model where health care providers who share a common vision work together to improve the health care provided to Medicare beneficiaries. The health care providers who participate in Arizona Care Network are voluntarily taking part in this Model with the goal of providing better quality care for our Medicare fee-for-service patients. All of the health care providers participating in our Next Generation ACO have joined with the Centers for Medicare & Medicaid Services (CMS) in committing to your needs and preferences, and to emphasize the coordination of your care. Privacy and Information Sharing
We put safeguards in place to protect your medical information because your privacy is important to us. Arizona Care Network is required to comply with the Health Insurance Portability and Accountability Act (HIPAA) and the Health Information Technology for Economic and Clinical Health Act (HITECH). If you have questions about the information Medicare can share with us regarding the health care services you receive under the Medicare program, or if you’d like to decline to have this information shared with us, please call us at 602-406-7226. Under current Medicare law, Medicare only covers care in a SNF if a patient has a prior 3-day inpatient hospital stay. The Rule Waiver available under the Next Generation ACO Model may allow beneficiaries to receive Medicare-covered SNF services at a participating SNF without a mandatory 3-day inpatient hospital stay.
